Ps : i've already post a thread for know who use max/msp but many people who have answer just use application based on it like pluggo :

do you realize presets on synths are often referred to as patches too? you might wanna add Max/MSP to your topic title ;)
i do tons of max/msp stuff, both at work as for my own audiovisual stuff. will post if there's anything i wanna share with the world. do you mean only audio stuff or more?
